To make the most of our consultation time, I ask that you complete this short intake form ahead of our call/meeting.
Your answers help me understand your current fitness level, lifestyle, goals, barriers, and any injuries or limitations so I can come prepared with realistic recommendations.
During your consultation, I’ll use this information to:
identify what type of training plan will suit you best (strength, fat loss, mobility, rehab-focused, performance, etc.)
suggest the best weekly structure for your schedule and energy levels
recommend the type of programming and accountability support that will help you stay consistent
explain which coaching option would be the best fit for you (in-person, online, hybrid, or self-guided)
